Please read and answer each statement. Please use an extra sheet if needed. The purpose of this questionnaire is designed to find out what you like and dislike about the Club currently. It is way for your Officers and Board members to make positive changes and provide additional services to the membership. Please complete the form and mail it to: Larry Smith 1207 Lakeview Drive Parkersburg, WV 26104. Yes this will require a few minutes of your time and 42 cents to mail it, but it will be worth the effort. Please return the completed form within the next week so we can put a plan in place.
